Chronic Disease Management

Chronic diseases are a leading cause of death and disability in Australia. Over half of Australians are at risk (Australian Institute of Health and Welfare) At Valentine Healthcare, our independent doctors are dedicated to managing chronic diseases. We offer personalized GP Management Plans (GPMP) and Team Care Arrangements (TCA) to support your health and well-being.

There are two kinds of chronic disease management plans for patients:

GP Management Plan
A GP Management Plan is a personalized care plan created with the guidance of our independent doctors to manage chronic diseases. Your GP will consider all aspects of your health, including medical, physical, psychological, and social needs. The plan is reviewed every six months or as needed based on your health status. Valentine Healthcare will notify you when it’s time to renew or update your plan.
Team Care Arrangement
A Team Care Arrangement works alongside your GP Management Plan to address long-term health conditions. It is designed to provide comprehensive care through a coordinated team of healthcare providers. Medicare covers Allied Health visits for eligible patients, making it easier for you to access the care you need. This approach is supported by evidence and is intended to support better health outcomes and overall well‑being.

Eligible Patients

The Key to Better Health

If you qualify, our nursing team will assist in creating your GPMP and TCA, including Allied Health referrals. This involves a thorough medical assessment and coordination with other healthcare providers. The process takes about 30 minutes, and you’ll receive your referral promptly. Eligible patients with a valid Medicare card can access this service at no out-of-pocket cost.

Consistent Care and Ease
Access up to five subsidised Allied Health visits annually, with a new plan available after 12 months. Our Professional GP Care Plans help you manage chronic conditions effectively, with our independent doctors guiding you toward better health and well-being.
