We offer personalised GP Management Plans (GPMP) and Team Care Arrangements (TCA) to support your health and well-being.
A GP Management Plan is a personalised care plan created with the guidance of our independent doctors to manage chronic diseases. Your GP will consider all aspects of your health, including medical, physical, psychological, and social needs. The plan is reviewed every six months or as needed based on your health status. Valentine Healthcare will notify you when it’s time to renew or update your plan.
If you qualify, our nursing team will assist in creating your GPMP and TCA, including Allied Health referrals. This involves a thorough medical assessment and coordination with other healthcare providers. The process takes about 30 minutes, and you’ll receive your referral promptly. Eligible patients with a valid Medicare card can access this service at no out-of-pocket cost.
